TELEPHONE ADVICE? If
you need to speak to a doctor for medical advice, please ring before 11.00am and
leave your details with reception. A doctor will return your call as soon as possible.
If the doctor you wish to speak to is not available and the matter is urgent,
the doctor on duty for that particular day will return your call.

MAKE
AN APPOINTMENT?
Consultations are usually 10 minutes long and are made through our receptionists,
by telephone or in person. Please refer to the list of surgery and clinic times
for details. If possible, please book with your usual doctor. Some problems do
take longer than others, so if a surgery does not run on time, please be patient.
Please remember to cancel your appointment if you find that you
cannot attend. If you are late you may be asked to make a new appointment.
The
practice recognises the right of the patient to choose a named healthcare professional
for general treatment or in relation to a particular condition (eg the choice
of a named female GP or treatment by a nurse for a particular condition such as
diabetes). Any such preference expressed by or on behalf of the patient will be
recorded in writing.
Patients between the ages of 16-74 who have not been
seen at the surgery for a period of three years may request a consultation with
a GP, at which any appropriate inquiries and investigations will be made. Consultations
can also be requested by patients over the age of 75 not seen at the surgery for
12 months.

with a number for SELDOC, our out-of-hours cover and the number for NHS Direct, a 24-hour helpline.
If you think you have an urgent medical problem
which cannot wait until the next available appointment, you will be seen by our nurse practitioner, who will
assess your condition, between 8.30 and 10.00am, Monday to Friday.
PRIVATE APPOINTMENTS
We also offer private consultations at the Clapham
Family Practice. For an appointment please call 020 7627 9630. Due to NHS restrictions, our registered NHS
patients may not avail themselves of these private consultations.

OBTAIN A HOME VISIT?
Home visits are made to
the housebound and patients who are genuinely too ill to attend the surgery. Please
give the receptionists as much information as possible as it helps the doctors
to assess the urgency of the problem. Patients are requested to telephone before
11.00am if a visit is required that day.

TEST RESULTS?
For test results you should
call our results line, 020 7498 1810, between 2.00 and 3.30pm, Monday through
to Friday. When you undergo any test it is your responsibility to contact us for
the result.
